To protect you and our staff during the annual respiratory viral season, from November 1 to April 30, and during sustained periods of high community transmission of respiratory viruses:
- Referral form from your general dentist
- Radiographs (X-ray films or images) from your general dentist
- List of any medications or supplements that you are taking
- Insurance information
- Patients under 18 years of age must be accompanied by parent or legal guardian

To protect you and our staff during the annual respiratory viral season, from November 1 to April 30, and during sustained periods of high community transmission of respiratory viruses:
- You will be screened for symptoms of airborne diseases within 48 hours of your appointment. If you cannot be reached for the pre-screening, your appointment will be rescheduled.
- When you check in for your appointment, your temperature may be taken and you will be screened for symptoms of airborne diseases. If you show symptoms of possible airborne disease, your appointment will be rescheduled.
- Please wear face covering to your appointment. Please do not use face covering that has an exhalation valve as this does not provide adequate protection for others around you. Please inform us at your pre-screening if you cannot use face covering due to medical reason.
- Please come to the appointment alone if possible. To facilitate social distancing in our office, any persons accompanying you may be asked to wait outside of the office or in their cars during your appointment.
- Patient under 18 years of age must be accompanied by a parent or legal guardian. Only one accompanying parent or legal guardian will be allowed in the office during your appointment.
- Patient who need assistance may be accompanied by an escort. Only one accompanying escort will be allowed in the office during your appointment.
- To facilitate social distancing, we do not double book. We only see one patient at at time. If you are late for your appointment, you may be rescheduled.
